COVID-19 Vaccine Clinic – June 26, 2021

The Sherwood School District, in partnership with Providence Health & Services and Washington County Public Health, is hosting a COVID-19 vaccination clinic at Sherwood High School on June 26, from 9:00a.m. to 1:00p.m. Vaccinations are free to everyone and no appointment is required. However, if you wish to schedule an appointment  ahead of time please visit https://washingtoncountycovidvaccine.as.me/SHS

The clinic will be administering the Pfizer vaccine to those ages 12 and older. To receive a vaccination, adolescents 12 to 14 need an informed consent form signed by their parent or authorized representative, as per state law. The informed consent form can be found here

As a second dose is required three weeks later, the vaccination clinic will open again at Sherwood High School on July 17, from 9:00a.m. to 1:00p.m.

The drive-up clinic will be held in the main front parking lot off Haide Road; please look for signage to direct you where to go. In addition to those driving to the site, walk-ups are also welcome.
