Routine health screenings are important to the student’s well-being.  The Sherwood School District offers several health screenings to our students.  The OAR 581.022.0705 states the school district shall maintain a prevention oriented health service program for all students that include vision, dental and hearing screenings.  Check with your child’s school office for specific screening type and dates.  


All screenings are free of charge

If you have any concerns about your child’s health, please contact your medical provider. If you do not have a medical provider or have financial hardship issues regarding your child’s dental, vision, or hearing health, please contact the District Nurses at the numbers at the bottom of this page.


Dental Screenings

Free dental screenings will be offered for students age seven and younger who have not been to a dentist. If you turned in a form to the school saying your child has already been to a dentist, your child will not be screened. This is a one-time screening.
IF YOU WISH TO OPT-OUT OF THE DENTAL SCREENING FOR YOUR CHILD, PLEASE CONTACT YOUR SCHOOL OFFICE.

This will be a screening only and should not be considered a complete evaluation. No treatment will be given to your child during the dental screening. Our volunteer Dentist/dental professional will look in your child’s mouth and make a note of any cavities or other dental concerns.

We will send the dental screening results home to the parent/guardian in backpack mail in case you need to follow up with your family Dentist. A copy of the screening results will also go into your child’s school file.

Hearing Screenings

Free hearing screenings are available for all first grade students.

This is a one-time screening. The screening is conducted by Portland State University graduate students from the Audiology Department with our Sherwood School District Speech Language Pathologists.
IF YOU WISH TO OPT-OUT OF THE HEARING SCREENING FOR YOUR CHILD, PLEASE CONTACT YOUR SCHOOL OFFICE.

Results from the Hearing Screening will be sent home to the parent/guardian in backpack mail.  A copy of the results will go in the student file.

Vision Screenings

Vision Screenings for Pre-Kindergarten through Fifth Grade

Free Vision Screenings are offered at Archer Glen, Hawks View, Middleton and Ridges Elementary Schools each school year through fifth grade.  The screenings are conducted by Oregon Lions Club with the help of parent volunteers using the SPOT screening device.  The screening is painless and is done from a distance and does not touch your child’s eyes or face.  Most children pass the exam and their “passing” results will not be sent home to the parent/guardian unless specifically requested.  For those children who receive a “not passing” (“referral”) result, the details of the screening will be sent to parents/guardians in student backpack mail.  A copy of the results will go in the student file.

THE PROVIDENCE PLAYSMART HEART SCREENINGS ARE SCHEDULED FOR Tuesday, November 14, 2023; 5:00-7:30p (PSVMC). Screenings are done at St. Vincent Medical Center (PSVMC), Providence Portland Medical Center (PPMC) and Providence Health Center – Reed’s Crossing (RC).
