Routine health screenings are important to the student’s well-being.  The Sherwood School District offers several health screenings to our students.  The OAR 581.022.0705 states the school district shall maintain a prevention oriented health service program for all students that include vision, dental and hearing screenings.  Check with your child’s school office for specific screening type and dates.  


All screenings are free of charge

If you have any concerns about your child’s health, please contact your medical provider. If you do not have a medical provider or have financial hardship issues regarding your child’s dental, vision, or hearing health, please contact the District Nurses at the numbers at the bottom of this page.

Hearing Screenings have been cancelled due to COVID issues.

351 students were screened in the 2019/2020 school year with an overall referral rate of 4%.

Free Hearing Screenings are conducted by Portland State University Audiology Department graduate students with assistance from Sherwood School District Speech and Language Pathologists.  Results from the Hearing Screening will be sent home to the parent/guardian in backpack mail. 
A copy of the results will go in the student file.

Vision Screenings have been cancelled due to COVID issues.

Vision Screenings for Pre-Kindergarten through Fifth Grade

Free Vision Screenings are offered at Archer Glen, Edy Ridge, Hawks View and Middleton Elementary Schools each school year through fifth grade.  The screenings are conducted by Oregon Lions Club with the help of parent volunteers using the SPOT screening device.  The screening is painless and is done from a distance and does not touch your child’s eyes or face.  Most children pass the exam and their “passing” results will not be sent home to the parent/guardian unless specifically requested.  For those children who receive a “not passing” (“referral”) result, the details of the screening will be sent to parents/guardians in student backpack mail.  A copy of the results will go in the student file.

Scoliosis/Posture Screenings


Our District Nurses had been giving free Scoliosis/Posture Screenings to 5th grade students, but after the 2019/2020 school year it was decided that in future we will do the Scoliosis/Posture Screenings in middle school.  When those screenings resume we will post the dates on this webpage and will provide the information to the middle school.  


PlaySmart Heart Screenings

Providence PlaySmart has cancelled all PlaySmart Heart Screenings at school settings through the end of this school year due to COVID issues. Please read further on how to schedule a PlaySmart screening at a Providence facility:

From January – August, 2021, Providence will continue the PlaySmart screenings for students age 12-18, but only at various Providence facilities, one per month, with 30 people being able to be screened at each session (must preregister).   

Youth Heart Screenings include FREE:
·     Electrocardiogram (EKG)
·     Blood pressure check
·     Echocardiogram, if needed

PlaySmart Heart Screenings for High School Freshmen (see note regarding other grades)


Free PlaySmart Heart Screenings are conducted by Providence Heart and Vascular Institute and consist of a blood pressure check plus an ECG/EKG.  This screening is painless and non-invasive. This is an “opt-in” screening:  we will only screen students who have a signed consent form (see below for forms). Students age 18 and older can give their own consent to be screened.
